End of the Year Report

Please provide the following information to help us document the outcomes of your project. This information will need to be provided to us by the date shown in your award letter.

PROJECT INFORMATION
  1. Please refer to your proposal funded by the Isora Foundation and list your goals and objectives. Describe the progress made toward accomplishing these goals.
  2. Did you make any changes to the project? Please explain any changes made to the project. Include changes in staff.
  3. Summarize the projectâ€TMs evaluation process? What were your outcomes? Provide your quantitative data. What did you learn from the evaluation?
  4. What do you consider the most notable accomplishments of your project this year?
  5. What do you consider the greatest challenges /obstacles faced in implementing your project?
  6. Describe any lessons you learned in carrying out the projectâ€TMs goals. Include lessons dealing with communities served, strategies, impact or unexpected results.
  7. Please attach any other information or material regarding your project that would highlight your projectâ€TMs activities such as pictures, brochures, new articles and other media.
