- Please refer to your proposal funded by the Isora Foundation and list your goals and objectives. Describe the progress made toward accomplishing these goals.
- Did you make any changes to the project? Please explain any changes made to the project. Include changes in staff.
- Summarize the project's evaluation process? What were your outcomes? Provide your quantitative data. What did you learn from the evaluation?
- What do you consider the most notable accomplishments of your project this year?
- What do you consider the greatest challenges /obstacles faced in implementing your project?
- Describe any lessons you learned in carrying out the project's goals. Include lessons dealing with communities served, strategies, impact or unexpected results.
- Please attach any other information or material regarding your project that would highlight your project's activities such as pictures, brochures, new articles and other media.
